Important Notice: Closure of Buckeye Health and Research

Dear Patients,

It has been our honor to serve you and your families. We regret to inform you that Buckeye Health and Research will be permanently closing its practice on December 31, 2024. After this date, we will no longer provide family or primary care services, including arranging tests, ordering new services, or refilling medications.

To ensure continuity of care, we strongly encourage you to arrange for a new healthcare provider as soon as possible.

Transition Options:

• Omnia Medical Group, conveniently located in the same building, has agreed to accept our patients who are in-network with their contracted insurance providers. Patients may choose to continue care with Ashley Brannan, Nurse Practitioner at Omnia. To establish care, please contact Omnia Medical Group directly at 614-587-7778.
• For patients choosing to establish care with other providers, we recommend consulting your insurance company’s provider directory or checking with local hospital referral services.

Medical Records:
To assist with the transition of your medical records, please contact our office no later than December 31, 2024. You may request records be sent directly to your new provider by completing a HIPAA-compliant authorization form. After this date, records will be securely stored electronically in accordance with local, state, and federal guidelines. Processing times for records may increase, and nominal fees may apply for hard-copy requests in accordance with .
