If you were a patient who had a visit with our practice within the last two years, you may have already received a copy of this letter by mail or in person. However, some letters were returned to sender due to address changes or being marked undeliverable. To address this, we are providing a copy of this notice as a courtesy to the public and to those patients who may have had an invalid address on file, have moved since their last visit, or did not receive the letter for any other reason.
